This Hospital based position will be responsible for providing Social Work Case Management and a full range of social work services including assessment, planning and therapeutic interventions and follow-up of high risk patients. Works well in an interdisciplinary environment consistent with the positions qualifications, profession practices and ethical standards. The incumbent shall also demonstrate accountability for and contribution to program development, quality improvement along with problem solving and productivity enhancement in a flexible interdisciplinary fashion.
