As our Social Worker, some responsibilities include discharge planning; planning, developing, organizing, implementing, evaluating, and directing the social service program to meet the objectives of the center as part of total patient care. Assist in meeting the psychosocial needs of residents and to enable residents to utilize medical care in order to achieve their optimal health.

Job Requirements:
Bachelor degree in social work, BSW, human services, or related field required.
Long term care experience required.
Must be able to meet the social, emotional, psychosocial needs of the Residents, be a team player, and part of an interdisciplinary team.
Have patience and tact, empathy.
Knowledge of Federal and State regulations helpful.
Must be able to multi-task be a quick learner, flexible, work independently and handle difficult situations.
