Job Description
This position is responsible for but may not be limited to:
Assisting members throughout the continuum of care when social indicators are noted
Identifies high-risk patients possible case management and social service intervention
Interfaces with providers of medical services and equipment to facilitate effective communication, referrals, development of discharge planning, and alternative treatment plan development.
Initiate contact with member/family, physician, healthcare providers/suppliers to discuss alternative treatment plan,
Conduct on-site evaluations as necessary
Evaluate each case for quality of care, documents, quality issues, and appropriate refers cases to Medical Director
Required Skills
Masters Degree in Social Work (MSW)
And/or Licensed Clinical Social Worker (LCSW), Licensed Masters Social Work (LMSW) , or Licensed Bachelor Social Worker (LBSW)
Three or more years in an acute care, managed care, social services community mental health center and/or hospital environment
For Behavioral Health, knowledge of Interqual Guidelines and State Mental Health Guidelines
