JOB DESCRIPTION: INTENSIVE IN HOME TEAM LEADER (Licensed)
REPORTS TO: INTENSIVE IN HOME PROGRAM MANAGER
DMHS SERVICE DEFINITIONS AND COMPONENTS FOR INTENSIVE IN HOME
Intensive In-Home (IIH) service is a team approach designed to address the identified needs of children
and adolescents, who due to serious and chronic symptoms of an emotional, behavioral, and/or substance
use disorders, are unable to remain stable in the community without intensive interventions. This service
may only be provided to individuals through age 20. This medically necessary service directly addresses
the recipients mental health and/or substance-related diagnostic and clinical needs. The needs are
evidenced by the presence of a diagnosable mental, behavioral, or emotional disturbance (as defined by
DSM-IV-TR and its successors), with documentation of symptoms and effects reflected in the
comprehensive clinical assessment and the Person Centered Plan. This team provides a variety of clinical
rehabilitative interventions available 24 hours a day, 7 days a week, 365 days a year.
PROVIDER REQUIREMENTS
IIH services shall be delivered by practitioners employed by mental health or substance abuse provider
organizations that
meet the provider qualification policies, procedures, and standards established by DMA;
meet the provider qualification policies, procedures, and standards established by the Division of
Mental Health, Developmental Disabilities and Substance Abuse Services (DMH/DD/SAS); and
STAFFING COMPONENTS OF AN IIH TEAM:
A. This Position: One FTE Team leader who is a licensed Professional who is actively involved in treatment and who supervises all members of the IIH team. (Provisionally licensed Professional who fills this position must be fully licensed within 30 months from the effective date of hire for this position.)
B. One FTE Qualified Professional who has the knowledge, skills, and abilities required by the population and age to be served (may be filled by no more than two individuals).
C. One FTE who is a Qualified Professional or an Associate Professional, and who has the knowledge, skills, and abilities required by the population and age to be served (may be filled by no more than two individuals).
D. For IIH teams focused on substance abuse interventions, the team shall include at least one Certified Clinical Supervisor (CCS), Licensed or Provisionally Licensed Clinical Addiction Specialist (LCAS), or Certified Substance Abuse Counselor (CSAC) as a member of the team.
E. The IIH team has a maximum caseload of 8 families.
ESSENTIAL FUNCTIONS AND OBJECTIVES OF IIH TEAM
IIH is designed to:

(through the age of 20 for Medicaid-funded services and through the age of 17 for State-funded services).
The service is intended to:
reduce presenting psychiatric or substance abuse symptoms,
provide first responder intervention to diffuse current crisis,
ensure linkage to community services and resources, and
prevent out of home placement for the child.
IIH services are authorized for one individual child in the family. The parent/caregiver must be an active
participant in the treatment. The team provides individualized services that are developed in full
partnership with the family. Effective engagement, including cultural sensitivity, is essential in providing
services in the familys living environment. Services are generally more intensive at the beginning of
treatment and decrease over time as the youths skills develop.
The IIH Team shall provide first responder crisis response, as indicated in the Person Centered Plan, 24 hours a day, 7 days a week, 365 days a year to recipients of this service.
ESSENTIAL FUNCTIONS AND DUTIES OF IIH TEAM LEADER
Providing individual and family therapy for each youth served by the team Designating the appropriate team staff such that specialized clinical expertise is applied as
clinically indicated for each child Providing and coordinating the assessment and reassessment of the recipients clinical needs Providing clinical expertise and guidance to the IIH team members in the teams interventions
with the recipient Providing the clinical supervision of all members of the team for the provision of this service. Develop and manage an individual supervision plan for all IIH team members Provide self help and living skills training for youth. Provide parenting skills training to help the family build skills for coping with the youths
disorder, and work with caregivers in the implementation of home-based behavioral supports. Crisis management, intensive case management, individual and/or family therapy, substance abuse linkage, skills training, and other rehabilitative supports to prevent the need for an out-of-home, more restrictive services. Conduct individual, family or group therapy sessions in accordance with the established person-centered plan. Ensures the participation of consumers in decision-making related to their service delivery. Provides consumers with clear and consistent expectations. Teaches consumers problem-solving skills. Provides information and techniques needed to rehabilitate issues associated with consumers mental illnesses, provides guidance including alternative treatments, and evaluates the effectiveness of treatment. Researches methods, techniques, and other sources of information to enhance service provision. Consults with physicians and other health professionals, as needed, to ensure a holistic approach to consumer care. Monitor and manage the presenting psychiatric and/or addiction symptoms Maintain confidentiality of consumer information. Complete training requirements and demonstrates ongoing competency in applicable areas. Maintain ongoing communication with supervisor and co-workers regarding significant or unique consumer treatment issues.

OTHER DUTIES AND RESPONSIBILITIES OF IIH TEAM LEADER:
Demonstrate an understanding of consumer rights and ensures rights protections for the consumer. Immediately report any concerns regarding the consumers, employees or agency to supervisor. Work closely with other Program Directors and Clinical Director to establish and maintain a continuum of client care. Fulfills any additional tasks and responsibilities as requested by the supervisor related to the needs of the agency. Review medical necessity for consumers, and assist in determining the appropriate level of service Ensure linkage to needed community services and resources Ensure that all services provided meet the Service Definition criteria
Ensure that all IIH Team members are able to complete the appropriate and required training as needed Ensure that all discharge criteria or continued service criteria is met as well as expected clinical outcomes for each recipient as defined in the Service Definition Ensure all documentation for requests for services and for services rendered and billed are accurate Maintain a positive attitude with consumers, co-workers, supervisors and external agencies. Attend required meetings provided by the agency, the local management entity and other applicable sources.
KNOWLEDGE, SKILLS AND ABILITIES
The occupant of this position shall possess the following knowledge, skills and abilities: Knowledge of time-limited intensive family preservation interventions intended to stabilize the living arrangement of consumers, promote reunification or prevent the utilization of out-of-home therapeutic resources (i.e., psychiatric hospital, therapeutic foster care, residential treatment facility) for youth through the age of 20. These services are delivered primarily to children in their familys home with a family focus. Knowledge of, and experience in, the provision of services to persons with mental health and/or substance abuse issues. Knowledge of community resources and services. Ability to work well with others and to present self in a professional manner. Strong oral and written communication skills. Flexibility in schedule and ability to adapt to changing needs within the agency and system of care at large. This is not an 8-5 Monday through Friday position. Many clients are not home or available during the day.
QUALIFICATIONS AND EXPERIENCE REQUIREMENTS
The minimum requirements of this position shall include one of the following sets of qualifications:
An individual who holds a license, provisional license, certificate, registration or permit issued by the governing board regulating a human service profession, or a registered nurse who is licensed to practice

in the state of North Carolina by the NC Board of Nursing who also has four years of full-time accumulated experience in mh/dd/sa with the population served.
TRAINING REQUIREMENTS (effective 1/1/11)
Within 30 days of hire to provide service, the IIH Team Leader must complete: 3 hrs IIH service definition required components 3 hrs of crisis response 11 hrs of Introduction to SOC
Within 90 days of hire to provide this service or by June 30, 2011 for employees hired prior to 1/1/11, IIH Team Leader must complete: 13 hrs of Introductory Motivational Interviewing 12 hrs of Person Centered Thinking Minimum of 24 hrs of training required by modality selected for service delivery (Cognitive Behavior Therapy)
Annually the IIH Team Leader must complete: Minimum of 10 hrs of follow up or ongoing training required by modality selected for service Delivery (Cognitive Behavior Therapy) All follow up training, clinical supervision, or ongoing continuing education requirements for fidelity to therapy(ies), practice(s), or model(s) must be followed (a minimum of 10 hrs annually)
ADDITIONAL REQUIREMENTS SPECIFIC TO INTENSIVE IN HOME STAFF:
Candidate must maintain licensure required for position; have a valid North Carolina Drivers license and a clean driving record that warrants insurability on Precisions Non-Owned and Owned Automobile Insurance Policy; must have proof of insurance on personal vehicle, and current registration at all times. This position requires going into homes of clients, working odd hours to enable time with the entire family of each client, and attention to detail documentation of services rendered. Some weekend work may be required and will be responsible for being on-call for crisis line in rotation with other Precision clinicians.
