Licensed Clinical Social Worker (LCSW) - Weekend Bookmark and Share
JOB SUMMARY:

Assists with coordinating patient care activities in the Emergency Department (ED) by facilitating assessments and screenings for appropriate disposition/level of care or discharge plan. May also assist with routine social work duties on hospital medical units as available. Completes an initial assessment of those patients meeting high risk or triggered screening criteria. Coordinates/facilitates admissions, transfers/referrals to appropriate behavioral health facility. Identifies opportunities to continuously improve quality and cost effectiveness of care and assists with monitoring as needed. Facilitates interventions for appropriate follow up/referrals for ED clients.

Follows all established organization and department policies, procedures, objectives and quality assessment. Responsible for assisting in maintaining a safe working environment throughout the organization by adhering to all safety management policies and procedures which reflect the mission and philosophy. Attends all mandatory in-service programs and maintains professional and education requirements.

JOB FUNCTIONS:

Essential Percent of Time

1.Conducts initial assessments/screenings, utilizing appropriate knowledge of the principles of growth and development over the life span of the patient, based on review of the medical record and contact with the interdisciplinary team, patient, family and/or significant other to identify appropriate level of care/ potential discharge needs.20%
Assists with recommendations for appropriate level of care or a discharge plan for patients on assigned units and assist with arrangements for implementation of the plan including, when necessary, verification of coverage for necessary resources. Treatment/follow up/Discharge planning for certain populations: ensure patients know what resources are available, assist with indigent prescriptions /treatment options, assisted living, shelter housing, transportation needs and identify referrals to health access.20%
2.Document contacts, interventions and appropriate information in medical record at the time of such activity.15%
3.Communicate with nursing, ancillary departments and physicians to facilitate patient care/appropriate level of care recommendations. Specific to Emergency Department Social Worker: patient visits on any length of stay over two (2) hours or on certain diagnosis-listening, helping explain time needed to interpret results. 10%

4.Participate in Social Service on call rotation as needed. Assist with social service needs on hospital acute units during night shift and weekends when not actively involved in ED cases. 10%
5.Complete and submit appropriate documentation/precert for inpatient admissions and for the placement/transfer of patients to outside facilities.10%

6.Compile discharge planning statistics to report to Health System departments and committees as needed. Maintain timely and current entries into the Social Services databases. Complete monthly chart review functions and daily call backs when necessary.
7.Maintain educational status to keep skills and licenses current.
8.Collect information from available resources, including physician offices and information systems to assist with the completion of the placement process.5%

JOB SPECIFICATIONS

Education Required:
Masters Degree in Social Work, Marriage and Family counseling, Counseling Psychology, Educational Psychology (not all inclusive, but must be related Master Degree)

Education Preferred

Registration, Certification and/or Licensure Required:
Indiana licensensure as Licensed Clinical Social Work (LCSW), Licensed Mental Health Clinician (LMHC), Licensed Marriage Family Therapist (LMFT) or related licensure.

Experience Required:
1 year experience in a hospital/healthcare environment

Experience Preferred:
1 year in a hospital or healthcare setting with discharge planning experience
1 year experience in Behavioral Health or Emergency Dept. work

2 years experience in Utilization Review and/or Case Management

Skills, Knowledge and Abilities Required:
Knowledge of age specific needs.
Ability to triage and assess clients in high acuity, high stress situations/environments
Extensive and current knowledge of governmental, local, and community resources.
Knowledge of governmental regulatory requirements pertaining to social services and discharge planning.
Ability to review medical records and information, and to make decisions based on clinical

indicators and identified patient needs.
Ability to maintain confidentiality of patient/family information.
Good basic math skills/ability to compile statistics.
Excellent verbal/written communication skills.

Able to work as a positive and professional member of a team.
Ability to govern own work load with multiple deadlines and time requirements.
Ability to use automated, clinical information systems.
Ability to maintain electronic databases.

Ability to maintain filing systems.

Additional Comments:
The Social Worker is a Weekend Option position working every Sat and Sun 7am-7pm.

It is a normal job for a RN but abnormal for a Social Worker. It is paid a great deal more because it hold a weekend option premium. Some people like the weekend option because they have their whole week free.

The person MUST have a License. Such as Licensed Clinical Social Worker.
