This position is responsible performing case management-related duties at the professional level in concert with the RN case managers to include clinical, financial, leadership and educational components. The major areas of accountability include Utilization Management, DRG Assurance, Care Coordination, Case management of specific populations, Discharge Planning, and Performance Improvement. The case manager role is critical to the appropriate utilization of resources and achieving quality and cost outcomes. The scope of the case manager role spans the continuum of care from pre-admission to post-discharge. Effective communication and collaboration with multidisciplinary team, including physicians, is integral to effective job performance.
The successful candidate must be able to perform daily Wyoming Medicaid Psych and Acute Care transfer review and performs weekly ICR reporting to Wyoming Medicaid; perform functions necessary to comply with CMS and other regulatory requirements relative to the case management and/or utilization management functions of the hospital; participate in performance improvement for dialysis patients under the direction of the Director of Quality; maintain a basic understanding of the function of each department and how each interacts with utilization management of resources, social services, and quality of care; perform and coordinate discharge planning, case management, and social service functions; contacts third party payers for LOS authorizations and continued stay certifications; minimizes discharge delays and the unnecessary use of acute care resources; participate on community committees; and more.

This is a 75+ bed hospital located in the southwest corner of Wyoming.
Job Requirements:
This position requires a Masters of Social Work with licensure in Wyoming as a Licensed Clinical Social Worker (LCSW), and BLS certification. At least 2 years of medical social work experience is essential. Experience in hospital case management and a minimum of one year of social work experience in dialysis is preferred. Must have knowledge of nursing process and performance improvement process.
