The Director of Case Management directs the activities of Case Managers, Social Workers as well as ancillary staff in coordinating health care planning, facilitation and evaluation of selected interventions across the continuum of care. Serves as a resource person to ensure medically appropriate, cost effective care. Actively participates in hospital performance improvement functions, particularly in the development and analysis of Clinical Pathways. Leads and participates in hospital committees designed to improve the quality of patient care, collecting and analyzing of data to identify patterns of care, resource utilization or other areas where process improvement actions can positively impact quality of care, cost, resource utilization and length of stay.

Job Requirements:
Current RN license in state of Texas
Bachelors required; Masters preferred
5 years experience in healthcare industry; current Director of Case Management preferred
CCM preferred
Job Schedule:
Daytime shift; flexibility as needed
