Job Duties
Responsible for assisting members throughout the continuum of care when social indicators are noted. Conduct on-site evaluations as necessary. Evaluate each case for quality of care, document quality issues, and appropriately refer cases to Medical Director.

Job Requirements:
Required Experience
MSW and/or LCSW, LMSW, or LBSW. Three or more years experience in acute care, managed care, social services community mental health center and/or hospital environment.
