Performs case escalation to the clinical team on the basis of a series of risk identifiers. Support providers in establishing on-going treatment plans for their members.

Job Requirements:
Required Skills
Bachelors degree in a social sciences field. Previous call center/customer service experience, strong communication skills and computer data entry skills. 3 or more years in a related field.
